| چکیده | ||
| زمینه و هدف: نارسایی مزمن قلبی (CHF) با کاهش برونده قلبی، کاهش قدرت عضلانی و اختلال در تعادل همراه است. مطالعات بیومکانیکی متداول اغلب بر متغیرهای گسسته تمرکز دارند و ممکن است بخش مهمی از تغییرات پیوسته و وابسته به زمان در چرخه راه رفتن را نادیده بگیرند. هدف از مطالعه حاضر، ارزیابی جامعتر تغییرات بیومکانیکی راه رفتن با استفاده از روش نگاشت پارامتریک آماری (SPM) و بررسی دادههای کینتیکی و کینماتیکی در فاز ایستایی راه رفتن در بیماران مبتلا به CHF در مقایسه با افراد سالم بود. مواد وروشها: در این مطالعه مورد-شاهدی، ۲۰ بیمار مبتلا به CHF با کسر جهشی کمتر از ۴۰ درصد، کلاس عملکردی II–III انجمن قلب نیویورک و توانایی راه رفتن مستقل، با ۲۰ فرد سالم همسانشده از نظر سن، جنس و جرم بدن مقایسه شدند. دادههای سهبعدی کینماتیکی و نیروهای عکسالعمل زمین طی راه رفتن ثبت شدند. زوایای مفصلی و گشتاورهای مفاصل ران، زانو و مچ پا با استفاده از روش دینامیک معکوس محاسبه و گشتاورها نسبت به جرم بدن نرمالسازی شدند. دادههای فاز ایستایی به ۱۰۱ نقطه نرمالسازی زمانی شدند و برای مقایسه منحنیهای سری زمانی از آزمونهای SPM{t} استفاده شد. همچنین، در تحلیلهای مربوطه، اثر سرعت راه رفتن با استفاده از تحلیل کوواریانس کنترل شد. یافتهها: بیماران مبتلا به CHF در مقایسه با افراد سالم، نیروی عکسالعمل زمین در راستای داخلی–خارجی بیشتری در بازههای ۱۸–۵۵٪ و ۸۵–۹۳٪ فاز ایستایی نشان دادند. اکستنشن مفصل ران در صفحه ساجیتال در بازه ۴۲–۵۳٪ و اداکشن مفصل ران در صفحه فرونتال در بازه ۱۰–۱۰۰٪ بیشتر بود. در صفحه عرضی، گشتاور چرخش داخلی مفصل ران در اوایل فاز ایستایی (۳–۸٪) بیشتر و در اواخر فاز ایستایی (۷۲–۹۰٪) کمتر بود. بهطور مشابه، گشتاور چرخش داخلی مفصل زانو در اوایل فاز ایستایی (۴–۱۰٪) بیشتر و در اواخر فاز ایستایی (۷۸–۹۰٪) کمتر بود. در مفصل مچ پا، گشتاور اورژن در بازههای ۲–۷٪ و ۱۵–۱۸٪ بیشتر، درحالیکه گشتاور چرخش داخلی در بازههای ۱۸–۲۶٪ و ۴۳–۸۰٪ فاز ایستایی کمتر بود. نتیجهگیری: بیماران مبتلا به CHF تغییرات معناداری در مکانیک راه رفتن در طول فاز ایستایی نشان میدهند که شامل افزایش نیروی عکسالعمل زمین در راستای داخلی–خارجی، افزایش اداکشن ران و تغییرات وابسته به زمان در گشتاورهای مفاصل ران، زانو و مچ پا است. تغییر جهت برخی از تفاوتهای گشتاوری در بخشهای مختلف فاز ایستایی نشان میدهد که تحلیلهای مبتنی بر متغیرهای گسسته ممکن است بخشی از الگوی واقعی تغییرات را نادیده بگیرند. روش SPM امکان شناسایی زمان وقوع و تداوم این تغییرات را در کل فاز ایستایی فراهم میکند. همچنین، در تفسیر تفاوتهای بیومکانیکی راه رفتن در این بیماران، در نظر گرفتن اثر سرعت راه رفتن اهمیت دارد. این یافتهها میتوانند در طراحی مداخلات توانبخشی با تمرکز بر قدرت اندام تحتانی، تعادل و کنترل عصبیعضلانی مورد استفاده قرار گیرند. | ||

| Background and Objective: Chronic heart failure (CHF) is associated with reduced cardiac output, diminished muscle strength, and impaired balance. Conventional biomechanical studies often focus on discrete variables and may therefore overlook important continuous, time-dependent changes throughout the gait cycle. This study aimed to provide a more comprehensive biomechanical assessment using Statistical Parametric Mapping (SPM) to analyze kinetic and kinematic data during the stance phase of gait in patients with CHF compared with healthy individuals. Materials and Methods: In this case-control study, 20 patients with CHF, characterized by an ejection fraction <40%, New York Heart Association functional class II–III, and independent walking ability, were compared with 20 healthy controls matched for age, sex, and body mass. Three-dimensional kinematic data and ground-reaction forces were collected during walking. Hip, knee, and ankle joint angles and moments were calculated using inverse dynamics, and joint moments were normalized to body mass. Stance-phase data were time-normalized to 101 points, and SPM{t} tests were used to compare time-series curves between groups. Where appropriate, gait speed was controlled for using analysis of covariance. Results: Compared with healthy controls, patients with CHF demonstrated greater mediolateral ground-reaction force during 18–55% and 85–93% of the stance phase. Hip extension in the sagittal plane was greater during 42–53% of stance, while hip adduction in the frontal plane was greater during 10–100% of stance. In the transverse plane, hip internal-rotation moment was greater during early stance (3–8%) and lower during late stance (72–90%). Similarly, knee internal-rotation moment was greater during early stance (4–10%) and lower during late stance (78–90%). At the ankle, eversion moment was greater during 2–7% and 15–18% of stance, whereas internal-rotation moment was lower during 18–26% and 43–80% of the stance phase. Conclusion: Patients with CHF exhibit significant alterations in whole-trajectory gait mechanics during the stance phase, including greater mediolateral ground-reaction force, increased hip adduction, and time-dependent alterations in hip, knee, and ankle joint moments. The direction of some joint-moment differences varied across the stance phase, highlighting temporal patterns that may be overlooked by conventional discrete analyses. SPM provides a useful approach for identifying the timing and duration of biomechanical alterations throughout the stance phase. Accounting for gait speed is also important when interpreting biomechanical differences in this population. These findings may inform rehabilitation interventions targeting lower-limb strength, balance, and neuromuscular control. | ||
